• 401(k) Wage Deferral Election
  Submit or update your 401(k) deferral election via the member portal.
   

• SUB to DC Allocation
  Choose where to allocate SUB contributions over $6,500 (extra $1.00/hour to HRA or DC) by completing the form in the member portal.
   

• Participant Enrollment Form
  Each participant must complete the 2026 Participant Enrollment Form by December 31, 2025, for 2026 claims to be paid. You can complete this form online or via the member portal.
   

• Spousal Insurance Requirement Form
  If you are married and your spouse works full-time: 

  • If their employer coverage costs less than $125/month, they must enroll.​

  • If it costs more than $125/month or there is no coverage, you must complete the 2026 Spousal Insurance Requirement Form and Employer Verification Form by December 31, 2025, to avoid disruption in coverage.
     

• Annual Wellness Visit - Lower Deductible for 2026
  Complete your annual wellness visit by December 31, 2025, to qualify for the lower 2026 deductible.
